Sherburne County Public Health is committed to the health and well-being of families in our county. Our Personal Nurses visit with families during pregnancy and the early years of parenting.
Pregnancy and a new baby can bring a lot of questions. We are here to help! Our visits focus on supporting parents to have healthy pregnancies and positive birth experiences. After baby is born, our visits help promote your child’s health, including providing newborn weight checks, breastfeeding and infant feeding support, parenting and safety tips, and education on how your child is growing, learning, and developing. These services are free of charge and visit times and locations are flexible.
If you are a provider or outside agency interested in making a Family Home Visiting referral on behalf of a family, you may fill out the referral form found on the First Steps Central MN website. You can still call (763) 276-0441) or fax the Referral Form to 763-765-4250. Please get the family’s permission prior to contacting us. We are happy to answer questions and help determine the best program for a family.
