Long Term Care Consultations

Many individuals struggle with the service options available to support them safely in their own homes. In Sherburne County, social workers asses each individual’s needs through Long Term Care Consultations (LTCC) and assist individuals in arranging supportive services that will provide safe and appropriate living options. This assessment is used for our seniors that are enrolled with the Blue Cross Blue Shield MSHO/MSC+ option.

MnCHOICES

MnCHOICES is a web-based application that is comprehensive and integrates assessment and support planning for people who need long-term services and supports in Minnesota. MnCHOICES embraces a person-centered approach to ensure services meet each individual’s strengths, goals, preferences and assessed needs.

Long Term Care Consultations/MNCHOICES give individuals the option to live in home and community-based settings with appropriate services to delay or prevent placement in institutional settings such as nursing facilities or hospitals.  

Accessing Services

You, or a representative who can speak on your behalf, can request a MnCHOICES assessment by completing and submitting the HHS Service Request Form or by calling the Sherburne County Intake Line at 763-765-4000 and choosing option 4.  Professionals can also make referrals through one of these two options.
