Booster Vaccines

Pfizer vaccine booster is the only vaccine approved by FDA/CDC.

3rd Dose Vaccine
Talk with your healthcare provider about your medical health, and whether an additional dose is appropriate for you. Your healthcare provider is likely the best place to receive your additional vaccination. Moderate to Severe Immunocompromised
  • Been receiving active cancer treatment for tumors or cancers of the blood
  • Received an organ transplant and are taking medicine to suppress the immune system
  • Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
  • Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
  • Advanced or untreated HIV infection
  • Active treatment with high-dose corticosteroids or other drugs that may suppress your immune response

Additional Information

For clinics involving children younger than 18:
Children under the age of 18 must have consent signed on their behalf by a parent or guardian when scheduling an online appointment. It is very important that the scheduling forms are filled out correctly in advance and to have a parent or legal guardian go to the vaccine appointment with their minor child(ren) to learn about the COVID-19 vaccine and provide consent for the child to receive the vaccine. For the Sherburne County Community Vaccination Sites: 
A parent or legal guardian must be present.  If staff are not able to obtain pre-registration parental consent or onsite verbal/written consent, staff are not allowed to vaccinate the patient.

Do not schedule an appointment if:

  • You have been exposed to COVID-19 and will be in quarantine during the clinic date.
  • You are positive for COVID-19 and may be in isolation during the clinic date.

Scheduling your appointment:

  • Do NOT schedule more than one appointment for yourself. 
  • Use the Chrome web browser if you have issues with the link.
  • When you get to the ‘Health Insurance’ screen, choose “No Insurance” in the ‘Insurance Type’ dropdown.  
  • Don’t stop at the Review Screen when registering!  Click Save and Continue at the bottom of each screen until you get to the confirmation screen that can be printed for your records.
  • If needed, you can cancel your appointment using a link at the bottom of your confirmation email.

The day of your appointment:

  • You should be feeling well.  If not, cancel your appointment or contact or 763-765-4156 to reschedule.
  • Before coming to your appointment, be sure to have something to eat.
