COVID-19 vaccination clinics

Clinic information:

Participants can schedule for their 1st or 2nd dose appointments as needed.  If scheduling a 1st dose appointment, you will get information while at our clinic regarding scheduling your 2nd appointment.  There must be at least 10 people signed up for each clinic or participants will be rescheduled.  If you have questions please reach out to us at 763-765-4156 or covid@co.sherburne.mn.us.

Thursday, 5/13/21, 8:45am-12:00pm at Sherburne County Government Center - https://prepmod.health.state.mn.us//appointment/en/reg/1161605692.  Moderna vaccine will be administered at this clinic. The 2nd dose will be due 28 days after the 1st dose. Participants must be 18 years or older.

Wednesday, 5/19/21, 1:00pm-3:15pm at Sherburne County Government Center - https://prepmod.health.state.mn.us//appointment/en/reg/1215916065.  Moderna vaccine will be administered at this clinic. The 2nd dose will be due 28 days after the 1st dose. Participants must be 18 years or older.

Thursday, 5/20/21, 1:00pm-3:15pm at Sherburne County Government Center - https://prepmod.health.state.mn.us//appointment/en/reg/9210216165.  Moderna vaccine will be administered at this clinic. The 2nd dose will be due 28 days after the 1st dose. Participants must be 18 years or older.

Tuesday, 5/25/21, 3:00-6:00pm at Big Lake High School on the Health & Human Services Wellness Bus -  https://prepmod.health.state.mn.us//appointment/en/reg/3516296006.  Pfizer vaccine will be administered at this clinic. The 2nd dose will be due 21 days after the 1st dose.  Participants under age 18 must have parental consent.T

Thursday, 5/27/21, 9:00am-11:00am at Sherburne County Government Center - https://prepmod.health.state.mn.us//appointment/en/reg/5931660612.  Moderna vaccine will be administered at this clinic. The 2nd dose will be due 28 days after the 1st dose. Participants must be 18 years or older.

Wednesday, 6/2/21, 9:00am-11:00am at Sherburne County Government Center - https://prepmod.health.state.mn.us//appointment/en/reg/3235913064.  Moderna vaccine will be administered at this clinic. The 2nd dose will be due 28 days after the 1st dose. Participants must be 18 years or older.

Thursday, 6/10/21, 9:00am-12:00pm at Sherburne County Government Center https://prepmod.health.state.mn.us//appointment/en/reg/6245913060.  Moderna vaccine will be administered at this clinic. The 2nd dose will be due 28 days after the 1st dose. Participants must be 18 years or older.

For clinics involving children younger than 18:

Consent Guidance: 
Children under the age of 18 must have consent signed on their behalf by a parent or guardian when scheduling an online appointment. It is very important that the scheduling forms are filled out correctly in advance and to have a parent or legal guardian go to the vaccine appointment with their 16- or 17- year-old to learn about the COVID-19 vaccine and provide consent for the child to receive the vaccine. For the Sherburne County Community Vaccination Sites: 
A parent or legal guardian must be present.  If staff are not able to obtain pre-registration parental consent or onsite verbal/written consent, staff are not allowed to vaccinate the patient. 

Do not schedule an appointment if:

  • You have been exposed to COVID-19 and will be in quarantine during the clinic date.
  • You are positive for COVID-19 and may be in isolation during the clinic date.
  • You have had any vaccine administered in the 14 days prior to the clinic date.
  • You are unable to attend the Dose Two clinic date.

 

Scheduling your appointment:

  • Do NOT schedule more than one appointment.  
  • Use the Chrome web browser if you have issues with the link.
  • When you get to the ‘Health Insurance’ screen, choose “No Insurance” in the ‘Insurance Type’ dropdown.  
  • Make sure you don’t stop at the Review Screen!  Click Save and Continue at the bottom of each screen until you get to the confirmation screen that can be printed for your records.
  • If needed, you can cancel your appointment using a link at the bottom of your confirmation email.

 

The day of your appointment:

  • You should be feeling well.  If not, cancel your appointment and contact covid@co.sherburne.mn.us or 763-765-4156 to reschedule.
  • Before coming to your appointment, be sure to have something to eat.
  • Wear a shirt that allows easy accessibility to your upper arm.  A t-shirt is great!
  • Please bring a photo id.
