Adult Services
Adult Services are available in these areas:
- Mental illness/Developmental Disability
- Chemical Dependency
- Adults in need of protection (Vulnerable Adults)
- Elderly in need of home care
- Persons in need of transportation
- Waivered Services for disabled adults under the age of 65
- Traumatic Brain Injury (TBI)
And the services we provide include:
- Adult protection; assessment and follow-up
- General assessment
- Case management
- Funding for support services based on assessed needs and eligibility
- Counseling
- Adult foster care
- Pre-petition screening; Mental Health & Chemical Dependency
- Transportation for the elderly, disabled, and general public
- General Volunteer programs, including R.S.V.P. (Retired Seniors Volunteer Program)
- Preadmission screening for nursing homes
- Meals for the elderly (Senior Dining Sites)

- Alternative Care Program - The Alternative Care program helps elderly Minnesotans pay for home care services so they can continue to live independently in their community.
- Care Call - The Care Call program provides a telephone contact to those who may have little or no other community/family contact on a regular basis. Care Call volunteers place a call to those requesting the service, to check on their welfare and provide reassurance. Calls are placed on requested days at a time prearranged between you and the volunteer. Care Call Volunteers will send someone to your home to check on you if you do not answer your telephone as expected. To arrange a Care Call, call your Sherburne County Senior Advocate at (320) 253-9349 or 1-800-333-2433.
- Homes Plus (Central Minnesota Council on Aging) - Homes Plus is a unique, affordable housing option for seniors in a home-like setting. Homes Plus is adult foster care homes licensed to care for seniors. They offer 3 meals, laundry, some personal cares, medication reminders and administration. Homes Plus also offers 24 hour supervision and much more.
- Senior LinkAge Line - The Senior LinkAge Line is a statewide program that links you the caller with the Area Agency on Aging where you live. This is a free telephone information and assistance service, which makes it easy for older adults and their families to find community services. If you are calling for information on the needs of someone not in your area, we will transfer your call to that region for specific information about the programs and services in the area they are living in.
- Volunteer Health Insurance Counseling Program - The Health Insurance Counseling Program assists callers with questions on Medicare and other insurance questions.
