There are many ways to receive services when you have temporary or long-term care needs. You may choose services that come to you, whether you live at home, at a seniors' residence or in other community settings. An example of services available to seniors would be house keeping or assistance with personal cares (i.e. bathing, dressing, etc.).
How do I access services?
Contact Sherburne County Social Services and request a Long-Term Care Consultation (LTCC) Screening. This screening is done by a Public Health Nurse and Social Worker in your place of residence to help identify the services you might need or want and also help plan those services. There is no charge for the screening.
