Long Term Care Consultations

Many individuals struggle with the service options available to support them safely in their own homes. In Sherburne County, social workers asses each individual's needs through Long Term Care Consultations (LTCC) and assist individuals in arranging supportive services that will provide safe and appropriate living options. Long Term Care Consultations give individuals the option to live in home and community-based settings with appropriate services to delay or prevent placement in institutional settings such as nursing facilities or hospitals.

Effective June, 2014, Long Term Care Consultations will change to MnCHOICES assessments in some circumstances.

What is MnCHOICES?

MnCHOICES is a web-based application that is comprehensive and integrates assessment and support planning for people who need long-term services and supports in Minnesota. MnCHOICES embraces a person-centered approach to ensure services meet each individual's strengths, goals, preferences and assessed needs.

MnCHOICES is for people of all ages who have any type of disability or need for long-term services and supports. The MnCHOICES assessment will replace current long-term care assessment processes and forms, including:

  • Developmental disability screening
  • Long-term care consultation assessment
  • Personal care assistance assessment
  • Private duty nursing assessment
